The CCM Approach
A CCM Medical professional meets with the patient at their home to conduct a non-invasive exam and also collects information on current medications and dosages, chronic conditions, care providers and answers questions regarding treatments.
A CCM Physician reviews exam results and works closely with medical specialists to optimize treatments. In addition, Chronic Care Services also manages complex paperwork for the patient, coordinates doctor appointments and optimizes pharmaceutical medications.
CCM is also on call 24/7 to the patient in case of a medical event or adverse reactions to medications or dosages. Additional patient services include remote monitoring of preventative and proactive care and treatments -- and monthly contact. Services are billed to the insurance company and Medicare with very little, if any cost to the patient.
